The C-peptide test is primarily used to evaluate hypoglycemia,
helping distinguish between endogenous insulin production (e.g.,
from insulin-secreting tumors) and exogenous insulin
administration. It is also valuable for assessing residual
beta-cell function in individuals with diabetes, especially when
insulin antibodies interfere with direct insulin assays. High
C-peptide levels with low plasma glucose can support the diagnosis
of an islet cell tumor.

In Type 1 diabetes, C-peptide levels are typically very low or undetectable, reflecting minimal to no endogenous insulin production. In contrast, patients with Type 2 diabetes often have normal to elevated C-peptide levels, especially in the early stages, indicating ongoing but often insufficient insulin production or insulin resistance.
